CalAIM: Enhanced Care Management & Community Supports

Coming soon.

Through California’s CalAIM initiative, CommUnify is expanding care coordination for Medi-Cal members with complex needs. Enhanced Care Management gives eligible members a dedicated care team to coordinate physical health, behavioral health, and social needs in one place — alongside Community Supports that address real-life barriers like housing and food.

We’re growing these services to reach new populations across the county in the months ahead.

Economic Empowerment · families with kids 0–5

Financial Coaching

Build the skills to get ahead.

For families with young children (ages 0–5), the Economic Empowerment program teaches practical financial strategies that build toward sustainable independence — daily money management, budgeting and savings, and understanding credits and debits. You leave with tools you’ll keep using.

Eligible Have a child 0–5 · SB County resident · willing to attend at least 4 sessions

Fall prevention · home safety

Senior Safe at Home

Free home repairs that help seniors stay independent.

Free repairs focused on health, safety, and fall prevention — so seniors can stay in the homes they love. Grab bars, handrails, smoke and CO detectors, heater safety testing, and slip-and-fall prevention. We also connect seniors to community partners for continued care.

79 households received free repairs and home upgrades in 2023 · average participant age: 75.

Utility Bill Payment Assistance

Keep the lights, heat, and power on.

Help paying your utility bills — so you can prevent a shut-off, restore disconnected service, or get ahead on what you owe, freeing up money for other needs. Assistance covers gas, electric, oil, wood, and propane. Intake centers are located across the county for easy access.

Weatherization Services

Lower bills. Safer, more efficient homes.

No-cost or low-cost home energy upgrades and education — designed for safer, healthier, more energy-efficient homes that cost less to run. Home safety testing, Energy Star appliance upgrades, air sealing and insulation, and heater or hot water repair or replacement.
